APPOINTMENTS

We realize that your time is valuable and we try our very best to stay on schedule. We ask you to assist us by:

  • Being on time for your appointment

  • Bringing your referral if required by your insurance along with your ID and Insurance Card

  • Completing your registration and history forms prior to your appointment (available below)

 

Minors must be accompanied by a parent or guardian. If accompanied by another adult, they should bring written permission giving authorization to treat in the presence of a responsible individual.

Please cancel 24 hours in advance to avoid any fees.

INSURANCE ACCEPTED

We participate with most of the major insurance companies in the Metropolitan area:

  • Aetna

  • Aetna Better Health (*Patient must be at least 5 years or age or older)

  • Amerigroup (*Patient must be at least 5 years or age or older)

  • Blue Cross Blue Shield 

  • Cigna

  • Humana

  • Medicare/ Medicaid

  • United Health Care (Commercial Only)

Appointment Cancellation Policy 

Scheduled office appointments which are canceled with less than 24 hours notification maybe subject to a $25 fee this fee will also apply to missed appointments. The cancelation and no show fees are the sole responsibility of the patient and must be paid in full before the appointment.
